Completing this form indicates your interest in participating in a Scottsdale Bible Church mission trip. If you have not previously served in any capacity with Scottsdale Bible Church, you will also be asked to complete a volunteer application. Depending on the trip you select, additional requirements may apply, and the Scottsdale Bible Church Volunteer Engagement Team will notify you of any next steps.

Please upload a picture of your current passport that clearly shows your full name, date of birth, passport number, and expiration date. If you do not have a passport yet or it expires within 6 months of your trip, please apply for one right away.

Most countries require your US passport to have a minimum of 6-months validity after your intended return date. Please review your expiration date to see if a renewal is needed. If you don’t have 2 blank pages remaining in your passport, not counting the Visa pages at the back, you will also need to renew your passport even if the passport hasn’t expired. To expedite a passport is currently taking 7-9 weeks, so plan accordingly.
